I've seen it in a few places but I have a few ideas that I'd be interested in seeing.
Possibly have America or England as the leader of the show and have everyone else be performers.
I particularly like the idea of having 1p/2p Nyo France's being Siamese twins.
Instead of making some things obvious have Feliciano or Romano be a strong man with Germany as an acrobat and Prussia a contortionist. Other backwards seeming roles would be great. (If you think of any comment what they could be!)
China could be the millions of years old guy still though cause jokes about his age are hilarious...
(I may add more to this idea later since I just came up with this now while laying on the couch dying cause I'm getting sick :P)
