So I took screenshots as I was making this cover, so it'll be a cover tutorial-ish :)

So we begin with a 654x1024 black or white image, it doesn't matter as it'll be unseen with the images merged on it.
Next, pick your model by adding a layer:

Then, using the smart brush, erase everything around the model; the gray. I tried using the magic brush, but unfortunately, it also includes her hair and top and erases those, so I stuck with the smart brush.
