in a form you should have: - title of story (optional) -subtitles (optional) - author name (optional) - theme (needed) ex: girly, purple, grunge, etc. include much detail as you want - images or characters - extra images/stickers
you can be as creative as you want describing the details! just be sure that they are clear so i don't have to ask questions about it later
