I realize because I haven't done these in a while that I don't really remember the format, so if it doesn't fit, just tell me. I'll take a day or two to put it back together correctly.
And, this is a strange request, but if you happen to use this cover, I would actually appreciate if you don't dedicate me. I don't plan to be active on here so I wouldn't want anything to really do with me anymore.
