The sheet can be confusing at times, so I made a key to give an explanation for some questionsand where you can get the information to answer them accordingly! I provided said information in the form of helpful links in the comments.

Character Sheet

:Identification:

One line characterization: Give a short description of their distinctive nature.
Ex: Amelia is headstrong and passionate; she stands up for what she believes and is always kind to others.

Full name: Give their current full name.
Ex: Amelia Anne Garner

Explain their full name:
• First name: Give the in-universe explanation for their given name.
Ex: Amelia: She has always loved the name, so she chose to use it instead of her previous name.
• Middle name: Give the in-universe explanation for their middle name.
Ex: Anne: Her grandmother's first name was passed down to her.
• Last name: Give the in-universe explanation for their surname.
Ex: Garner: her husband's last name that she took when she married him.
• Maiden Name: Give the in-universe explanation for their maiden name.
Ex: Copper: It was her father's surname passed down to her.

Origins of their full name:
• First name: Give the in-universe onomatology of their first name.
Ex: Amelia is Latin, comes from the name Amal, and means 'work'.
• Middle name: Give the in-universe onomatology of their middle name.
Ex: Anne is a Hebrew name, meaning gracious and merciful.
• Last name: Give the in-universe onomatology of their surname.
Ex: Garner is the reduced form of Gardener.

Previous Name: If their name has been changed in-universe, give their previously used names/aliases.
Ex: Mary, 'Paisley Jane'

Explain their previous name: Give the in-universe explanation for their previous name/alias.
Ex: First Name: Mary: Her mother chose it from a book of baby names.
Ex: Alias: Paisley Jane: Amelia's pen name; she secretly writes sci-fi/mystery novels.

Origins of their previous name: Give the in-universe onomatology of their previous name.
Ex: First Name: Mary was derived from an ancient Hebrew name, Miriam.
Ex: Alias: Paisley is Scottish and means 'church or cemetery'. Jane as a surname came from its given-name counterpart: Jane. It's derived from the Hebrew name 'Johanan', meaning "Graced by Yahweh".

Current Nicknames: Give any current nicknames/aliases.
Ex: Sparkles, Sharp-eye

Explain their current nicknames: Give the in-universe explanation of their current nicknames/aliases.
Ex: Sparkles: Her friends call her this because Amelia is usually seen in glittery outfits.
Ex: Sharp-eye: During a spy game as a kid, this was her alias (even though she doesn't have particularly exceptional eyes), and is now used by her family as a joke.

Previous Nicknames: Give any previous nicknames/aliases they had.
Ex: Guppy,  Greeny

Explain their previous nicknames: Give the in-universe explanation for any previous nicknames/aliases they had.
Ex: Guppy: Her father used to call her this when she was a toddler as she loved to swim.
Ex: Greeny: Her grandmother called her this because she loved gardening with her. It's based on the saying 'Green Thumb'.

Birthdate: Give their 'in-universe' (pertaining to any original dating/time system you may have made) birthdate.
Ex: Sunday, July 3rd, 1994
