Klaus Mikaelson broke Arabella's heart eighteen years ago. She suffered but survived. She had to, for her sons. Then she found love and comfort with Stefan Salvatore and they were happy. They got married and he died. As simple as that. Stefan was a hero. He wanted to protect the people he loved. His children, including the non-biological ones. Then there was Kai Parker from the Gemini coven. His older brother twin of Josette Parker slaughter his siblings. Olivia, Luke, Jo, and Kai being the only survivor aside from their father and John, Jo's twin brother.
There was this fate thing that everyone knew Stefan and Arabella had. It started with Silas and Arabella two thousand years ago. He loved her but he thought Qetsiyah had actually killed her. And then in 2012 they reunited once again. He wanted to take the cure to be with his Bella and when he realized she was alive he wanted to be with her but before they could even talk about it Amara killed him.
It took Arabella and Stefan a while before they got together. She didn't want to be with him because he reminded her of Silas. So they started fresh. They both left town to avoid the drama. They even bought a house and everything. They returned for Jo's and Alaric's wedding. That was the first time she encounter Kai who instantly took a liking to the girl. Of course she only saw him as a friend and nothing more since she was in love with Stefan. John killed mostly everyone at the wedding and by some miracle Kai survived. Damon killed John because he linked Bonnie to Elena.
Arabella ended up carrying the future leaders of the Gemini coven. Kai and Alaric were there through her pregnancy and believe of not Kai and Alaric were pretty good friends and really close. When Stefan left to run from the huntress Rayna Cruz, Kai was there being the friend and shoulder to cry that she needed at that moment, especially after giving birth. Kai had untold feelings for her. He loved her but she loved someone else.
Kai was undeniably in love with her but she only had eyes for Stefan and he knew it. So Kai stood right by her as her best friend when she got married. He wiped her tears and comforted her when she needed comfort. He had lost too much he and his nieces were the only three people left from the Gemini coven. He knew what it was like to loose someone that you loved. They didn't officially get together until a couple of years later but she did have a few night stands with him that resulted on their twins, Liam and Aurora.
(So Stefan died in 2015 and Liam and Aurora were born in 2017 after a one night stand)
Now, they had been married for almost ten years. Stefan died in 2015. Kai and Arabella had a one night stand where Aurora and Liam were conceived. Even though at the time Kai and Arabella weren't together they showed love and attention to their twins. They didn't make it weird or awkward or anything like that. In the contrary they acted like adults, except for Damon who was always throwing hints that they should be together.
And it wasn't that he didn't miss his brother or was trying to make her date someone else to forget his brother. Damon knew how much Arabella loved Stefan and she was always gonna love him. But Damon always told her that she had the right to be happy and move on with her life. She knew it, she just wanted to take her time and not rush anything. To which Damon would reply "you did rush. You skip the date, relationship, marriage and went straight to the honeymoon and kids". The kids call him uncle Damon and or uncle D. But he calls himself "the best uncle" which always ends up with Kol arguing with him about who's the favorite.
Kai has always been there for Arabella since the moment they met. He had told everyone to never call him for anything because he didn't want to be involved with anything that had to do with the supernatural world. But when Katherine and John returned and Arabella called he quickly came to the rescue, helping Bonnie send John to a prison world and Katherine back to hell. Silas was her first love, Klaus was her second love, Stefan was her epic love and Kai is her last love, her true love.
