It's very hard to become one. But there are some rare cases in who a hollow is born one. Kumagawa Misogi is such a case. And his former appearence was quiet demonic.

For it he even won a title. The Devil of Hueco Mundo. One of the reasons was that not even the king of Hueco Mundo couldn't kill him. Well, after that he stoped bothering him.
But there were others who were still doing that. Of course he anihilated every one of them.
