What Do You Mean By Form? Also, To Do An Adoptable Raffle (This Is Only My Way Of Doing It, Someone Else Might Do It Differently), I Have Three Or More People Enter For The Raffle (Example: You Might Need To Enter A Name Or Something For It) Also, To Enter, A Person Should Have To Enter A Password To Enter The Raffle, This Password Will Be In The Rules So Everyone Entering Reads The Rules, Then You Randomly Pick The Winner Of The Adoptable! (If I Need To Explain More, Just Tell Me!)
