I feel like Izuku hides his pain behind the smile because all the bullying, suicide baiting, being attacked by the sludge villain, and villain attacks. I feel like if he bottles them up he may blow up one day. The show is not going into izuku's angst about his past. I feel like everyone needs therapy in the show. I feel like izuku's smiles are fakes and he wants someone to noticed his pain.
