Basically Boyfriend was psychically, mentally, emotionally and verbally abusive to Pico during their relationship and helped shaped the trauma that made him the man he is today, which is why Pico doesn't kill Boyfriend, after all those years, he's still scared of him. The reason Pico's voice is that way in FNF is because he started smoking to relieve the pain from Boyfriend's abuse, his addiction and trauma only got worse after the school shooting. Mom and dad know Boyfriend's past of abuse towards his partners so that's why they want him dead and away from his daughter. Boyfriend got brain damage after Girlfriend's parents set him up for an incident that was supposed to kill him and became a better person and not did not end up becoming to Girlfriend, although he was definitely less intelligent after the brain damage. He doesn't fully remember his and Pico's abusive relationship but does know some parts. I'm putting this here because people argue that Boyfriend's too naive and innocent to be an abuser and Pico would be the one who was abusive. Now, If you've ever seen Pico's School, he was very innocent and cheerful before the shooting, he'd definitely be the type to let people abuse him, but he eventually was tired of being pushed around and snapped.
