Personally, I just think of the craziest and most far out idea out there, one that I know if I were reading I would be like holy crap. And then take that crazy insano idea and water it down a little so that it is more realistic and fit it into your story at the most unexpected time. Also you can allude to it subtly and I mean like so subtly that barely anyone picks up on it, but there are serious clues that that person or this event or whatever is going to happen :) I hope I helped!!
