Sukuna probably won't be entering the picture anytime soon. In this AU, he actually lived a happy, fulfilling life and died of old age, with no regrets and no grudges against the world. Because of that, he never entered a binding vow with Kenjaku in the first place. That's why Kenjaku keeps hunting Kyubi down every century. He believes she's hiding Sukuna's remains somewhere and wants to steal them from her.
So if Sukuna ever does return, it won't be through Yuji or any other vessel. He'd have to be reborn naturally. Whether he and Kyubi would even recognize one another after so many lifetimes is another question. As Kyubi once told Gojo, that's simply the nature of reincarnation.
As for children, there will be little chaos gremlins! A bunch of little kids running around with random combinations of Gojo and Foxie's personality traits, raised among geese, would be… concerning for everyone involved. I can imagine some poor neighbor showing up to complain about the "brats," only for Gojo and Foxie to have to clarify whether they're talking about the children or the geese.
I also like to imagine Foxie taking the kids to the school all the time and casually recruiting Tengen as a babysitter. Tengen would spoil them rotten. Meanwhile, the kids would be running wild around campus, clinging to Nanami's legs, adding at least three new stress migraines to Ijichi's weekly quota, and sneaking into Shoko's lab to mess with her experiments.
The funniest part is that one of them would inevitably become the first person in history to actually understand Shoko's explanation of RCT. At that point she'd immediately recruit them to teach future students because she's given up trying to explain it herself
