I saw some workers uncoupling a formed work use for casting in a construction site.
As a HSE Officer I quickly noticed that they we operating in an unsafe manner, I walk up to them and make necessary correction, then left the scene.
Few minutes later, I heard an accident alarm, unfortunately a formed work had fell on one of the workers.
It was there I noticed that they bluntly ignored my advice, and someone was badly injured.
