One of the animatronics have violently bit a person in the head. It have bit the frontal lobe of the victim's brain. The cause of death are blood loss.
Foxy the Pirate
People he might be the one who bit the person because he have sharp teeths, not like others. And at Pirate Cove, there is a sign said ' Sorry ! Out of order.'. Foxy himself is in a state of disrepair, possibly meaning that after the incident, the attraction was closed and abandoned. Noticeably, Foxy's jaw seems to hang loosely, as if it were damaged through excessive force.
Freddy Fazbear
Some speculate that Freddy himself was the animatronic responsible for the Bite. On the posted rules in the East Hall, one of the rules listed states, "Don't touch Freddy." This rule may have been enacted after the Bite. There are also two instances of faint handprints or hand-shaped impressions on Freddy's face, implying that he may have been pushed off of someone, pulled away from someone, or that someone may have been otherwise trying to struggle against him.
