Their parents want to admit them to a mental hospital but they cant as they are an adult (18 or just emancipated). Their therapist thinks the drugs are working cause Y/n is lying to Corthawl (Yes that Corthawl [let me know if I spelt it right]). They also hates their family, who have set names so you dont have to associate yourself with their strong distaste for their blood.
