So Sasuke was somehow sent to the moon while fighting Kaguya and couldn't figure a way out, he tried to see if he could contact the Sage of Six Paths but found that he couldn't, so he hoped that Naruto could do fine on his own until he came back.
He went exploring and found a temple that seemed to be kept in amazing shape as he begun exploring it. He walked into a room full of dead body's and saw that their chakra was that of Otsutsuki's. Meaning that this was once an Otsutsuki temple when a White Haired, Pale Skinned woman pushed him against a wall to which he instinctively switched their positions.
"Who are you? I know your an Otsutsuki judging by your chakra."
She opened her eyes and Sasuke saw the Byakugan peek out at him confirming that she was an Otsutsuki.
"My name is Toneri Otsutsuki, and judging by your chakra you appear to be Indra's reincarnation correct?
"You are right."
Sasuke saw no point in lying about something that she already knew.
She asked why he was here and he explained that he was fighting Kaguya when he was sent here. She agreed to try to help him leave this place so she shared her chakra with him.
Sasuke attempted to open a portal back to Kaguya's dimension but nothing showed up, a quick check and he realized that his Rinnegan was powered down and that his chakra levels were low. He cursed quietly at this as he realized that he would be stuck here for a while.
He tried to think of something but found that he was out of options, so he simply sat on a chair and waited for his Rinnegan to recharge. Toneri simply watched him from her side of the room that he appeared in. As he waited his mind wandered to other places as he thought about how much did Toneri's robe hid her figure.

On the date that he was told to come, he did. He opened a portal to the moon and when he arrived Toneri's stomach appeared to be huge and she was laying on a bed with her puppets taking care of her. Sasuke out the pieces together since it was roughly 9 months since their encounter and realized she was giving birth.
So he helped her through it and by the end of it, she gave birth to Itachi Uchiha. A boy who looked exactly like Sasuke, except for the shaggy, white hair that he inherited from his mother.
Sasuke and Toneri both agreed that Itachi would stay with her for a year before Sasuke would keep him.
