Percy was heart-broken. They had a third war when he was 35. Annabeth had already died with his daughter, during child-birth, when he was 29. All his friends and family were slaughtered. All the mortals were wiped out. All minor gods were forced to fade. The Fourteen Olympians were all weak and chained up. They made him their heir. The war was against most of the Primordials who were rebelling against Chaos. The Olympians sent Percy to Chaos for protection. Chaos was so shaken-up that he had started fading. He also made Percy his heir. 7 days later, Percy was suddenly filled with power and he fainted from exhaustion.
When he woke up he saw the universe being rebuilt by the Primordials. He found an abandoned planet and trained there with all his new-found powers. He also discovered 4 other forms of his. All 5 forms of his were :
1. Percy Jackson - Normal form. Looks normal with aura hidden. Black hair and kaleidoscopic eyes. Height : 6'2. Age : 21. Male.
2. Sirena Jackson - Female form. Longer hair till hip. Perfect figure that would put Aphrodite to shame. Same eyes and hair (color-wise) as mortal form. Height : 5'6. Age : 19. Female.
3. Chaos - Primordial form. White short hair with colorful highlights. Obsidian black eyes with flecks of other colors. Height : 6'4 or 40ft. Age : 26. Male.
4. Deus - Roman form. Brown short hair with forest-green eyes. Muscled and strong figure. Scars visible. Height : 6'3 or 20ft. Age : 32. Male.
5. Theos - Greek form. Black hair and sea-green eyes. Looks just like Poseidon. Muscles with swimmer's build. Height : 6'3 or 20ft. Age: 30. Male.
It took him millenniums to finally control all his powers. He then used his Chaos powers to explore the multi-verse. Finally, he found a universe were the gods used to have a minor influence and where he had a few family members. He teleported there. Earth-16. Home of the Justice League and weirder.
