Koku ran to. Douma with his sword. Everyone else in the room ran screaming away, while Douma tried to block koku's attack. That didn't work and only did, that his hand was cut off.
Koku continued to attack Douma, till he was only a big chuck with a head, without arms, legs and also without dick.
And that is the real story, how Douma, upper 2, really died! Not beyond Kanao and Inosuke. No, because he told the others that koku fucked Muzan.
