Not all death scars are permanent. Only ban scars, scars that come fresh after a revival, or scars from dying in battles that were important to the person. For example, Planet has a scar from the battle at Jaron's revival, because it was an important event to them.
Some scars that don't fall under these three categories will likely either fade over the season or just completely disappear at the start of a new season.
They can be symbols or just markings of where the wounds that caused them to die were. Sometimes both.
Currently Planet's biggest scar is a giant trident symbol, right under the scars where the prongs stabbed him on his back from the s4 finale. It runs pretty much all the way down his back.
He doesn't have any large scars from s5 yet, and most big ones came from his fights when he was working with Gucci Gang and Pirates when Ro died. Since they were some of the more important ones to them. But because they don't need to fight as much or do so out of desperation, he has much more s4 scars than s5 scars.
The majority of s5 scars so far are temporary ones that have either begun to fade or will disappear in the next season
