"Congratulations, you're now carrying the Ryomen heir."Megumi feels relieved to hear it was successful and he didn't have to get artificially inseminated once more. If he had failed this first time, he would've surely been punished for being a failure of an omega. "I'm going to step out a moment to call the boss, I'll only be a moment." The man with long, dark hair and a tattoo across the bridge of his nose informs Megumi. He was waiting for him when Megumi arrived at the doctor's office to meet with the private doctor of the Ryomen family, wearing dark clothes and carrying an intimidating aura. The doctor excuses herself as well to give Megumi some privacy.
With a trembling hand, Megumi brings his phone out of his back pocket. He calls his father first, holding back tears because he knows better than to show weakness. It rings twice before it's answered, "Hey baby, what's going on?"
Megumi's bottom lip quivers briefly, his voice cracking, "Daddy..."
"Dumpling? What happened? Are you hurt?" Toji questions with genuine concern. Megumi thinks his father might just be the only person in this unfair world that truly loves him. The man, even when he was at his worst, cherished Megumi and did everything he could so Megumi didn't go hungry or cold.
"I'm pregnant." Megumi answers, barley managing to pull himself together, but he does. "It was successful, I'm sure... that will please the elders."
It's silent on the other end as Megumi tells his father. The man had his suspicions about what was going on, but he was kept in the dark and to avoid any further hatred towards his disgraced father, Megumi didn't tell him either. He knew Toji would've fought with the elders of the family, refusing to let them use his son like every other respectable family treats their omegas. As nothing more than wombs to be sold off to the most eligible alpha.
The man sighs, "Oh baby... why didn't... why didn't ya tell me? Ya don't need to protect me; my job is to protect my kid. I didn't want ya being used as a damn tool for those bastards. Dumpling, ya had hopes an' dreams..."
"I know... but that's not how things go. I can protect you too. This... is a one-time deal with the Ryomen family. Just... one heir, then I can come back home. When I do... will you make my favorite for dinner?" Megumi clenches his hold onto the phone, already feeling homesick. If it were up to him, he'd have his disgusting healthy smoothies every morning that Toji makes and continue to be spoiled and pampered by the man who finds his purpose in caring for his little pup. Movie nights, cleaning day, even just grocery shopping with the scary looking man brings Megumi happiness because Toji loves him unconditionally. He's the only one who doesn't expect anything back from him, who does these clumsy yet sweet things without demanding compensation.
"Of course, dumpling. Then I'm never lettin' ya outta my sight again." Toji sighs, sounding hurt and exhausted. Megumi knows he's going to rain hell for the elders but there is nothing that can be changed, what's done is done. There is already a little one growing inside Megumi's body. He had wanted to be a spoiled kid for just a little longer, just... a little longer, he wanted to be spoiled by his father, but the elders wouldn't allow it. He's of age now and of the, arguably, best bloodline. Many wanted to get their grubby hands on him but Sukuna, who had recently taken over, had won him.
"That's a promise... don't... don't forget." Megumi hangs up because if he has continue talking to his father, he's going to break down into tears and he can only be comforted with a warm embrace from Toji who will pat his head and tell him that everything will be alright because they have each other.
The door opens, the doctor and the suited man entering once more. "The boss said to bring you back right away, unless you're hungry."
"I'm alright." Megumi murmurs, rubbing the side of his phone with his thumb. He thinks of himself as a bit silly for being scared, if it were up to him, he never would've gotten pregnant so soon. He would've waited a long, long time because, isn't it just a scary thing? Or is him thinking that proof he is just some pampered, naive pup?
"Make sure to listen to your cravings, don't be shy." The doctor tells him with a polite smile.

Megumi's mother had died giving birth to him, it was the same for her mother and so forth. The omegas of the Fushiguro bloodline were weak and there were always complications during the birth. Megumi thinks it would be a real tragedy if he wasn't able to go back home with Toji, if he dies like everyone else on his mother's side. He doesn't want Toji to be alone, especially not when it took so long for Toji's mental health to get good enough just to stop him from being so reckless with his life that Megumi never knew if Toji would come home or if he would be lying dead on the streets for an amount of money that's worth less than his life.
Omegas have a purpose. Male or female, they give birth. They produce their unique pheromones, produce slick, and go into heats monthly. Though some omegas have different heat occurrences. They age slowly much like alphas, staying youthful and healthy-for the most part. Alphas are born superior, as if they won the genetic lottery. Strength and intelligence, they are capable of achieving great things. That could also lead to ego problems. They produce intense pheromones and are drawn to omegas, and they can knot, sizes ranging.
Betas are average in every way. Looks and abilities, they have a very mild scent without pheromones, they can detect pheromones but barley. There are more of them than Alphas and Omegas, since those two secondary genders are naturally superior.
However, despite Omegas being a superior secondary gender, they have to deal with mistreatment from Alphas far too often. Betas are naturally sweet towards Omegas; it's said it must be some biological component that makes them behave this way. But that hasn't been proven, so Megumi ignores that little bit of information.
When the elders of the Zen'In family, Toji's side of the family, came to him with their arrangement, he had wanted to refuse. But Toji had already been mistreated his entire life by them and shunned because he was different. He fell in love with Megumi's mother and left with her, he wanted to cherish her sincerely and devotedly. The elders didn't understand him and saw him as a defiant, foolish man. He struggled so much but he felt it was all worth it, for her and for their child who would be born into a loving home.
Then his mother died and the only reason Toji hadn't followed beside her, was because of Megumi. Toji couldn't bring himself to abandon such a little, fragile infant with those innocent eyes. Till Megumi was six years old, all he knew was his dad. He never even considered having other family members. He wished that they never knew of his existence, if they hadn't then they never would've come and continued to ruin Toji's life.
"I'm Choso, by the way." The man introduces as he opens the car door for him. Megumi nods, getting into the passenger seat. Megumi continues to fidget with his phone throughout the drive, reminding himself of all that the elders told him. To keep out of the way, submit and obey, do not cause anyone trouble and everything should go smoothly. All he has to do is give birth to the heir, then he can go home. That is the deal, that is the promise.
Megumi is already homesick and even though nothing could be gained from it, he wishes that Toji would come take him back home already.
