The sound of bustling footsteps and distant voices filled the air at Seattle Grace Hospital, a symphony of urgency that was both familiar and comforting to Derek Shepherd. But today, the noise felt different. It was louder, more chaotic, as if the universe was mocking him for the chaos swirling in his own life.

Derek stood by the nurses' station, leaning against the counter and absently tapping his pen against the clipboard in his hands. He couldn't focus on the charts in front of him; his mind kept wandering back to Meredith. It had been just a week since they had officially broken up, yet the memory of her lingering smile, her soft laughter, and the way her eyes sparkled when she was passionate about her work haunted him.

He looked up as Miranda Bailey approached, her expression all business. "Shepherd, I need you in OR 3 in five," she ordered, her tone clipped. "We've got a neurosurgery case coming in, and I need your expertise."

"Sure, I'm on it," he replied, forcing a smile as he pushed away thoughts of Meredith. He had to focus. He was a surgeon, after all, and he needed to set a good example for the residents.

As he walked toward the OR, he caught a glimpse of Meredith in the distance, her brow furrowed in concentration as she reviewed a patient chart. His heart ached at the sight of her. They had been through so much together, and the sudden void left by their breakup felt like a gaping hole. He wanted to reach out, to say something, but every time he tried, the words got stuck in his throat.

Just as he was about to turn away, he noticed a woman standing beside Meredith—someone he hadn't seen before. She had long, dark hair pulled back into a neat bun, and she wore scrubs that looked fresh and crisp. There was an air of confidence about her that immediately drew his attention.

"Dr. Shepherd!" a voice called, breaking his train of thought. It was Bailey again, this time more insistent. "You need to get in there now!"

Derek nodded and hurried to the OR, trying to shake off the feeling of unease that came from seeing Meredith with the new woman. He scrubbed in, his mind racing through the procedure they were about to undertake. As the surgical team gathered, he tried to focus on the task at hand, pushing his feelings for Meredith deep down where they couldn't interfere.

The surgery was intense, requiring every ounce of concentration. Derek felt a sense of purpose as he navigated through the delicate procedure, guiding the younger surgeons with a mix of authority and encouragement. But in the back of his mind, he couldn't shake the image of Meredith standing there, laughing with the new doctor.

When the surgery ended, he stepped out into the hallway, feeling the weight of the world on his shoulders. He needed to talk to Meredith, but the thought of facing her filled him with dread. Would she still feel the same way? Would she hate him?

"Hey, Dr. Shepherd!" a voice called, and he turned to find the new neurosurgeon walking toward him, her expression bright and open.

"Dr. Campbell, right?" he asked, recalling her introduction from earlier that day.

"Exactly! I just wanted to say that was an incredible surgery back there," she said, her enthusiasm contagious. "I've read so much about your techniques. It was amazing to see them in action."

"Thanks," he replied, surprised by her sincerity. "You did well in there too."

Kate smiled, her eyes sparkling. "I appreciate that. It's great to be here. I've heard so much about Seattle Grace, and the stories are all true—it's a fantastic place to work."

"Yeah, it has its moments," he said, trying to keep the conversation light. But even as he spoke, he felt a pang of guilt for enjoying her company. He shouldn't be so easily distracted.
