I woke up this morning after a very little amount of sleep after a long shift last night in the ER. Chief Webber told me about a new surgeon starting today so I must make a good impression. I walk out of my bedroom stretching and yawning and see my best friend Chloe walking out of the bathroom half asleep.
'Morning y/n...' she said
I nod and walk into the bathroom and do my morning routine. I walk out and get dressed and drive me and Chloe to the hospital.
'Morning Chief Webber!' I pipe up in the elevator.
'Good Morning Dr. Holt. Dr. McDermott'. I make my way to the residents lounge and see Dr Bailey.
'Hey Miranda!' She nods and walks out to meet her new interns. I change into my scrubs, grab my pager and start off the day in the lobby as the Chief wants to introduce the new doctor.
'Good Morning Seattle Grace I would like to introduce Dr. Derek Shepherd, our new head of Neuro.' He says walking back to his office as Dr. Shepherd steps forward.
'I am very happy to be starting here at Seattle Grace. I hope you all give me a nice warm welcome.' Derek says.
I start to walk to do rounds but realise I have new interns.
'I have five rules. Memorize them. Rule number one. Don't bother sucking up. I already hate you and that isn't going to change.' I point to the collection of things on the desk. 'Trauma protocol, phone lists, pagers. Nurses will page you. You answer every page at a run. A run, that's rule number two. Your first shift starts now and lasts 48 hours. You're interns, grunts, nobodies, bottom of the surgical food chain, you run labs, write orders, work every second night until you drop and don't complain!' I get to the on call rooms and open the door. 'On call rooms. Attendings hog them. Sleep when you can, where you can, which brings me to rule number three, if I'm sleeping, don't wake me, unless your patient is dying. Rule number four, the dying patient better not be dead when I get there, not only would you have killed someone, you will have woken me up, we clear?' I say as I finish telling them about the on call rooms. Then a blonde haired, short intern raises her hand.
'That was only four rules, you said five?' She says. My pager beeps.
"Rule number five, when I move, you move!" I start to run to the ER. The paramedics come in with a knife to the chest and a head injury.
'Molly Duncalf, found at a car crash near one of the schools. Fragments of car to the chest, unknown head injury.' The paramedic said as they started to wheel the patient into the hospital.
'Trauma 1. Right, names?'
'Meredith Grey.' The girl who asked the question about the rules raised her hand again.
'Cristina Yang.' The Asian black haired woman says.
'Izzie Stevens' The blonde friendly intern says.
'George O'Malley' The male intern says.
'Grey, page neuro. Yang, order an OR. O'Malley, run tests.' I say trying to make sure the patient gets the best care possible.
'Where do you want me Dr Holt?' Izzie asks.
'In the pit. Go! Go! Go!' I say as Yang, O'Malley and Stevens leave the room. Grey pages neuro and helps me with the patient.
As I am treating the patient, trying to make sure I don't pull out the fragment and cause her to bleed out. The woman's sister, Ally comes to see her.
'We were on the way to pick up my daughter. I got out the car to go get her and then a car crashed into the side of Molly's car. Oh my god!' She moves and pulls out the fragment but I can't get there in time to stop her. Molly starts to bleed out. I call a code blue and proceed to save her life.
'Excuse me, nurse. Yeah, can you get her out of here please?' I say multitasking as my interns and two Attendings appear in the room.
'Dr. McDermott what happened?' Dr Shepherd and Doctor Burke come running in.
'The patient's sister pulled out a fragment of the car that was in her chest and now she's bleeding out.' I say as I try to stop the blood from flowing out.
'Get me a unit of O-neg and book and OR, I think the fragment may have been caught on her heart and lungs.' I jump on the bed to make sure the wound is blocked and she isn't bleeding out anymore.
I get to scrub in on the surgery and make sure she's okay.

I go out to Ally and tell her that Molly is fine and take her to the room.
'Thanks for saving her Dr. McDermott' Ally says sitting next to Molly.
'A little tip, don't ever pull out a knife or anything sharp from your body unless you want to bleed out, but, you're very welcome. It's what I do all day, every day.' I leave Ally in the room and finish Molly's chart.
'Good job in that OR today Dr. McDermott.' Dr. Shepherd runs up to me.
'Thanks Dr. Shepherd.'
'Which specialty are you going to pick?'
'I was thinking either, Neuro or General surgery.'
'Good idea. How would you like it, if you were on my service tomorrow?'
'Really?' Derek nods. 'Oh my god. Thanks so much. I'm looking forward to it.'
'See you tomorrow Dr. McDermott!'
'You too Dr. Shepherd.' I smile and place Ms Duncalf's chart near the nurses and go to the resident lounge to get changed. What an eventful day at work. New interns, crazy sisters and just standard surgical resident stuff. And Derek....
