This is about a person who has agateophobia (a fear of insanity). This person's fear of insanity leads him/her to insanity. With that said, the reason he/she get so freaked out by his/her reflections is because this person sees them as monsters.
At the end it says, ".. a million reflections there to observe me." A million is a hyperbole. The reflections are actually doctors, and to him/her, it's monsters. So basically ".. a million reflections there to observe me." means that there are doctors, all around the insane man/woman, observing him/her.
