pls read very carefully
- if you are not going to use the cover, please DO NOT request! i spend time on these and if you aren't going to use them, that's just a complete waste
- if you have asked for more than 5 covers (for different books), i will most likely NOT make them since i have lots to do - unless we're like friends and i like you :)
- forms that aren't filled out properly or are very brief and only include a few sections might affect my decision to make your cover. please fill out the whole form and make sure that it is clear
