Suga is the adult Emil. As you can see Suga is a little more effected by his little self. He gets on a piano and play a rhythm that was played in Begin, referencing to his younger self.
In the short film, Suga looks like if he was intoxicated and sluggish. In Demian, In the winter I believe, Sinclair visited his parents after being in boarding school. His mom was quiet unpleased with Emil and how he looked. She said that he looked "drunk and grey."
During this chapter it also expressed how Sinclair says that he sacrificed himself to get of this evil hole he fell onto. When the car rode past Suga and Suga ran upon the piano, this may signify how his sacrifice worked.
