You start out as a man named Jack on a plane, something happens and the plane crashlands near a lighthouse, inside the lighthouse, you find a bathysphere. After you get in the bathysphere you're shot down under the ocean to the beautiful city of Rapture, which upon closer inspection has fallen into disrepair. After the bathysphere docks, an Irishman named Atlas helps you find safety. The man who built Rapture, Andrew Ryan thinks you're an agent from the surface and uses the city's defenses and the pheromone-controlled "Splicers" against you. Atlas tells you that the only way to survive is to use the unnatural abilities that Plasmids give you by injecting them into your bloodstream, they alter your DNA and give you powers such as lightning and fire, he also says you have to kill the "Little Sisters" and their giant protectors known as "Big Daddies"  as seen on the chapter picture above, and extract their ADAM. You also meet a lady named Dr. Tenenbaum who tells you to save them instead by giving you a Plasmid that kills the embedded slug in them that turns them into little sisters, they then turn back into regular little girls. Atlas tells you that his wife and child are hiding on a submarine and directs you towards it to save them, right when you get there though Andrew Ryan destroys it and Atlas, enraged sends you to go kill Andrew Ryan. When you find Andrew Ryan in his office, he's just playing golf, he reveals to you that he pieced together the fact that you are his son who was born in Rapture only four years ago and genetically modified to mature rapidly. He also says that a man named Frank Fontaine bought your embryo and designed you to obey anything said after the phrase "Would you kindly?". You were then sent to the surface when the Rapture civil war started to put you beyond Andrews reach. When the conflict between Andrew and Fontaine got to a stalemate, you were sent instructions to board a plane and to use a revolver to hijack and crash the plane near the lighthouse, which made him a tool to Fontaine. Since your Andrew's son, you have access to the bathysphere system which was locked down to everyone except for the people with Andrews DNA. Ryan has you kill him, wanting to die on his own terms, After he dies you figure out that Atlas has been using the trigger phrase the whole time to control him, and then reveals that he is in fact, Frank Fontaine who faked his death to throw Andrew off the trail and take control of Rapture, and leaves you at the mercy of the city security systems. Dr. Tenenbaum and her little sister help you escape through the vents where you fall and blackout.

When you wake up Dr. Tenenbaum got rid of some of the conditional statements (like the trigger phrase) and helps him get rid of the others, which includes one that would have stopped your heart. After that, the Little Sister help you find Fontaine who injects himself with a ton of ADAM, turning himself into a human monster. After you weaken him enough so the Little sisters can extract all the ADAM from him.

Three endings are possible depending on how the player interacted with the Little Sisters, all narrated by Dr. Tenenbaum. If the player rescued all of the Little Sisters (therefore saving their lives), the ending shows five Little Sisters returning to the surface with Jack and living full lives under his care, including their graduation from college, getting married, and having children; it ends on a heart-warming tone, with an elderly Jack surrounded on his deathbed by all five of the adult Little Sisters.

If the player harvested (and therefore killed) all of the Little Sisters, the game ends with Jack turning on the Sisters after defeating Fontaine, presumably killing them all and taking their ADAM. Tenenbaum narrates what occurred, condemning Jack and his actions, voice thick with anger and contempt. Later in the second ending, a ballistic missile submarine carrying a nuclear missile comes across the wreckage of the plane and is suddenly surrounded by bathyspheres containing Splicers. The Splicers kill all hands aboard the sub and take control of it. If the player saved some of the Little Sisters, but killed a fair few as well, the ending is visually identical to the second one, though the tone of Tenenbaum's voice is a sad one, as opposed to an angry one.
