1: I think the biggest problem people had with Milo was that he didn't use his mind link when Dallas was attacking him. The chapter after the incident explained why, as Blaiz had asked Milo.
So, Milo was being seriously attacked, he was going to be raped and killed. He was under a lot of stress and wasn't thinking properly. He had been having such a good time the last few weeks that he let his guard down. He forgot about his mind link, which is why he didn't link Blaiz. Some people are GREAT at keeping their heads on in a crisis, however, Milo is NOT one of those people. Milo has had a hard life, his growth was stunted, he had been betrayed by his own BROTHER. Honestly, Milo was not alright at all. Milo had deep psychological problems at that point.
2: Another issue people had. How did a house full of werewolves not hear Milo being raped and abused?
As Milo stated at the beginning of the book, when he would try to mind link anyone in the pack for help, it would be blocked. This could have been due to his psychological problems. He could have unintentionally blocked the link when he was being raped and abused. He could have unintentionally blocked it because he didn't want anyone to know. Along with not wanting anyone to know, he wouldn't have made much noise while it happened. It was also mentioned that Milo was gaged during most of these times.
Another thing, Zachary could have used magic to block the mind link, and he could have used magic to make the room sound proof.
