Ivan sat in the car as he was driven to a mental hospital. Yekaterina looked at him sadly in the mirror as she drove.

Once they reached the hospital and he was taken in and evaluated. They said that he needs to stay for a while. That they'll get him a bed and send him up.

When Ivan was sent to his room in the hospital he was mistreated there as well.

What the other patients had done to him were cruel things, they would spit at him, hit him, yell at him, throw things at him and take his food and belongings. When they stomped on his scarf that Yekaterina had made him...he broke down and grabbed the scarf from them and went to the laundry room to wash it. He cried as he stood there. Why were they so mean? What did he do? Was it just his existence? That he will never know.
