Miles visits the Springer residence while investigating about the murders, there he finds Otto Heckel trying to steal some valuables from the house. The police suddenly arrive at the residence.
Otto tries to make a deal with Miles ,that he will lead him to the person who killed the Springers if he accepts to work with him as an underground surgeon for criminals.
Miles and Otto go to the killer's apartment. There, he sees a man with a gun, who tells him that he befriended Johan and that he was invited to the Springer residence several times. One day, the Springers decided to destroy a bed of sunflowers to build a parking spot. Since the man tells that he used to walk with his mother in a sunflower field, he told Johan -then called Erich- that this was a terrible thing, Johan agreed and told the man that he was no longer part of the Springer family and asked him to kill his foster parents.
When Miles is about to leave, the man tells him that Johan left a message for him at the Springer residence. Then the man shoots himself.Miles and Otto go back to the Springer residence where they find Johan's message written on the wall.
"My dear Dr.Miles , Look at me ! Look at me ! The monster in me has grown so big !"
Otto tricks Miles into a medical gig consisting of saving a terrorist's life. Miles refuses to treat Karl Brandt(The terrorist).Finally he agrees to do some emergency procedures and tells his partner to take him to an hospital.
Otto then betrays Miles and leads the police to the terrorists hideout after seeing on Tv that there is a bounty on both of them.
Miles and Karl both leave the hideout and he tells Karl that he saved him because he was human.
