The Book Of Quirk Malfunctions!

2 (Katsuki Bakugou)

Bakugou is messing around with Kirishima and Kaminari and goes to make a small explosion in Kaminari's face, but Kaminari turned Bakugou's hand towards Bakugou, and his quirk malfunctions, causing a huge explosion right in Bakugou's face. Kaminari backs out of the smoke and goes over Kirishima. Kirishima calls out to Bakugou, surprised to get no response. Once the smoke finally clears out, they find a headless Katsuki Bakugou, his hand resting beside where his head would've been, a splatter of blood flowing from his neck. Kirishima and Kaminari begin sobbing over the loss of their friend, knowing there's no way for Recovery Girl to fix this.
