Leader position will go to the slot with the highest leadership score among those who said they want to be leader, if no one says they want to it will automatically go to the one with the highest, note that MG1 doesn't have a leader.

The form here is very simple, if you're chosen to debut then you will get a form to fill with fun info that will be good to know for debuting Idols ^^ also for debuted/debuting Idols you can always PM the CEO (aka me ;) ) if you want to add/change anything.
