The Sicilian mafia is largely what inspired Passione. They eventually expanded to the US via a splinter widely referred to as the American Mafia, which is now an independent ally of its organization of origin. Both are referred to as "Cosa Nostra" ("Our Thing") by its members.

The Irish and Italian Mafia had been fierce rivals. For the easiest explanation, I'll use the Winter Hill gang and American Mafia (or just Cosa Nostra) for words. The Winter Hill gang was one of the most successful organized crime groups in US history, and as previously mentioned, the American Mafia is a split off of what is essentially Passione.

WH operated during the 90s and early 2000s before disbanding.
But what if they hadn't?
Furthermore, let's say AM didn't become somewhat independent from CN(P) either.

[When referenced in tandem, I will refer to the Sicilian and American Mafias as Cosa Nostra (CN) from this point forward. ]

Surely WH would take every opportunity to destroy CN. And Giorno eliminating CN(P)'s drug trade, which would deal a crippling blow to CN(P)'s income, would be that opportunity.

So here's what I'm thinking.

The WH gang, but the protagonist(s) (OC, reader, self-insert) run it.

Upon catching wind of Giorno eliminating the trade, they make plans to take out the core leaders of CN(P) themselves. A few AM bosses dissapear, and flights are booked to Naples upon word from their sOuRcEs that that's where Giorno is located, and shit hits the fan.

It's not a perfect idea, but who gives a shit. Coincidentally, this AU was how Dolley was born.
