as for coming up with ideas, they usually just come to me. whether i'm watching tv, listening to music, looking out the window, going through the "what if?" sort of thoughts--they kind of unravel as i'm going about my normal day. one time I got the idea of this fantasy story from a combination of three dreams of mine. xD
when it comes to planning, it really depends on the story for me. if it's a fantasy book that needs an in-depth knowledge of the world my characters will be encountering, then I take more time to plan. with books like faking delinquency, I tend to write as I go along c:
