THIS may come as a surprise; but there is no true form ! I'm giving you all the freedom to make your own forms but I do indeed require a form. You can use your own form or a revised one if need be as long as it gives me a good amount of information on your characters
as for requests, just simply write down the gender, age, breed, and scenario ideas in the comments on this page ; if there are any special attributes you want them to have please let me know (:
