Germany still has nightmares about WWII, and the Nazi's, and all the horrible things he did to the Jewish population. He'll scream at night and no one can save him. He cries himself to sleep most night nights because of the guilt in his head. But he hides it all from Italy because he doesn't want him to worry about him. Italy's smile is the only thing that keeps him going. This is also the reason why Germany trains Italy so hard. Because if he snaps again, and kills yet again, he wants Italy to be able to surivive it, and protect himself against him.
