Prior medications: Zoloft (non-compliant)

Prior admissions: n/a

Prior Diagnosis: Anxiety, Depression, SI

Patient arrived to MH from the ED at UMass Medical Ctr. At that time patient was combative until sedated for transport.

From the Emergency Department: ED staff reports patient is physical well. Received several doses of Haloperidol with good result for agitation. Tox screen upon arrival to ED showed high levels of Sertraline, Diazepam, and Acetaminophen.

HX of: Prior suicidal ideation reported by parents, anxiety, depression, and episodes of panic, most recently the day of her disappearance.

Pt disappeared from her home on 10/ /94, leaving no note. Was found ________ days later nearby her home inside the entryway of the _________________ hospital by an off-duty police officer. Required medical attention on the scene and was transported to UMass Memorial Medical Center in stable condition. Given charcoal upon arrival with good result. Patient awoke in the ED screaming for an individual named Ryan Murphy. Insistent on locating and could not be subdued without medication. All Ryan Murphy's in the area located and unknown to the patient.

ED MD Watson signed medical clearance for patient to be transported to Milton on 11/3/94.

Psychiatry Notes:

Given the high profile of this minor patient, and the ongoing nature of this case, family asks that privacy be of the utmost concern. Also, given this pt's previous noncompliance with medication, this physician recommends IM administration of this medication until she is stabilized. Pt should be on 1:1 supervision per 24 hours. Recommendations for further treatment going forward prior to release: compliance of medication x30 days, group therapy 1xday, 1:1 therapy 1xper day, hypnotherapy 3xweek, and involvement in activities on the unit.

I wake up gasping for breath every morning. Every time I wake up and see the white walls, white sheets. They wouldn't even let me go home for a day before they brought me here.

"Samantha. Honey. We've looked into it. Really, we have. There is no Ryan Murphy at that address. No Ryan Murphy who matches your description. Do you think... was there someone else with you those days before?"

Memories that the medication has blurred come flooding back. Ryan's face. That terrible beautiful place. I feel her hand brush up against mine as she walks by, a small piece of paper moves into my pocket. I feel as if I can't breathe and struggle to move my legs to get back into my room before I sink behind the door and pull it out of my pocket.

He's still there. We have to help him."

Everything comes flooding back in a wave that overcomes me as everything goes black.
