Facing death.
Trying to acquire a hocrux of the Dark Lord is definitely a way to die. He expected it, not making it out alive. It was inevitable. He was going to die young anyways. Though, he does feel bad for leaving Kreacher alone.
Not Sirius. Sirius would've happy instead (Because who would be sad if a death eater died?), especially due to their fight that day. Because why would Sirius worry for Regulus when he said himself that James Potter is his only brother?
"I don't have a brother except for James! James is my only brother." Sirius shouted after Regulus had practically begged him to return for Orion's funeral. He doesn't blame him for not wanting to.
(But it hurts. What he said hurts Regulus.)
(Everything wasn't supposed to turn out like this. Sirius was supposed to be the perfect little Black heir and Regulus the spare.)
(So why did it turned out like this?)
He should've taken Sirius' offer to run away. If he had, maybe he could've been a family once more. (With a few additions.)
But alas, fate is fate. And no one can defy it.
