This is when Haruto is in a "game" that is real. He gets attacked by monsters, along with a girl he met named Silicia. Silica ends up getting badly injured- to the point of death. If Haruto does not get back fast- or heal her fast- she will die.
This is Haruto in another universe... Kind of a crossover.
