Glenville Sanatorium, West Virginia, 1974

Somewhere in West Virginia, in the middle of nowhere on a mountain, a sanatorium hospital stands on the mountain, surrounded by snow and trees. Inside are orderlies, doctors, nurses, and security guards. They're working on patients who are mentally ill, emotionally unstable, and are deformed. Walking down a hallway are two doctors, and a new nurse. Dr. Brendan Ryan, the chief of staff, and his assistant, Dr. Alan Campbell. The nurse is Dr. Ann Marie McQuaid. They're walking to a security door.

Dr. Ryan: We have 182 patients. Most are horribly deformed, due to inbreeding and birth defects. Ward "C" is high security. Each patient in here should be considered dangerous or a danger to himself. No one can get in or out without first passing through this door. We can open all the cells at once in case of fire, but the protocol is one cell open at a time.

He knocks on the door. A buzz is heard, and the door is unlocked. Dr. Ryan opens it, and they walk in. Dr. McQuaid closes the door. They walk past a guard.

Dr. Ryan: Thanks, Charlie.

Charlie: No problem, boss.

He unlocks the door for them, and they all walk in the ward. The ward has eight cells. On both sides. Dr. McQuaid looks at this as the patients in their cells are chattering. A man is heard laughing in his cell.

Dr. Ryan: I know what you're thinking. But we have to do this for their own good.

Dr. Campbell: He's right. Believe us, we need to do this. Because these people need so much help.

Dr. Ryan: This is not a prison. We hope to rehabilitate the patients if at all possible and have them rejoin the general population.

They walk down the aisle as Dr. Ryan looks at the list.

Dr. Ryan: Before any cell is opened, you must have an orderly--

Just then, they both hear Dr. McQuaid screaming. They turn, and see a patient grabbing her by her hair. They both rush to her, pushing on him, to make him let go.

Dr. Ryan: Bobby! Bobby! Let go!

Dr. Campbell: Bobby, let her go!

They push him, making him let go of her hair. They check on her.

Dr. Campbell: You okay, Dr. McQuaid?

Dr. McQuaid: Yeah. That just surprised me. That's all.

Dr. Campbell: We told you not to get too close to the cells.

Dr. Ryan: Yes. You shouldn't wander past the yellow lines. They can and will grab you from their cells.

Then, he takes them to a cell, where three young patients are in.

Dr. Ryan: These are the Hillicker brothers.

One of them shouts as he runs at the bars, making Dr. McQuaid gasp and jumps back. He looks at them, and walks back to his brothers. He has his left eye removed, like it was cut out. His youngest brother is sucking on his hand, and his oldest brother is sitting on a bench, shaking around while wearing a mask.

Dr. Ryan: We call them that because Mrs. Hillicker found them in the woods, standing over the partially eaten remains of what we think were their parents. And the little freaks are smart. Definitely the most dangerous patients in the hospital.

Dr. McQuaid: They're just kids. And the most dangerous?

Dr. Ryan: Yes. Trust me. Dr. Campbell?

Dr. Campbell: Yes, sir. See the youngest one, the one sucking on the stump of his hand?
