A family curse

Fairly obvious.

Origionally from Greek mythology. The story goes:

There once was a king named Lycaeonia who sacrficed meat to the god Zeus. A fairly common occurence in Greece, expect Lycaeonia used human meat. This displeased Zeus, who turned the king into a wolf. The king's name is the origin of the word 'Lycanthrope' and the idea of being turned into a werewolf through curse has been passed through the ages.

The curse could be placed by a sourceror, demon, preist or god.
