S/C/A/R/E/C/R/O/W Citizen Mental Evaluation
For: Battery City Recovery Center
Date: 5.10.2019Citizen: Calix A. Mansfield
Interview Conducted by: Dr. Susan Hamilton
At: Clearwater Hospital
_______________________During the brief interview conducted, it was quite obvious that the patient had no desire to be admitted to the hospital. She asked many times if there were other options of recovery. She also claimed multiple times that her mother had over exaggerated the severity of her condition. She said that the main reason of not wanting to stay at the recovery center was "mainly the clothes I'd have to wear." She described the clothes of the patients she saw as being "unfashionable, not colorful enough, and too generic."
Due to the citizen's choice of statements and opinions, it is believed that she has the makings of a rebel. This should be solved during her stay at the recovery center.
When being questioned about the claims her mother made, she didn't exactly deny most of them; most of the time she counteracted the questions with hypothetical theories, which is a reason to believe most of the claims were true. If this is true, the patient has experienced minor anxiety during schooling, when meeting new people, and around BL/I workers. Other suggestions of mental illness include but are not limited to: self harm, disordered eating, change in identity, and violent thoughts.
Her other mental facilities are in working order (coordination; motor skills; understanding of time and place.) An exception to this was when she had referred to herself using different pronouns. She completed highschool and was set to attend the Battery City College.
For this patient, an extended stay in the rehabilitation center would be sufficient, as we could evaluate her more. She should be put into minor suicide watch due to her past of depression. She may need a higher dosage of mood stabilizers, and an addition of different medications. She should be with other patients, as this could improve her stay, her social anxiety, and depression.
