Quick author's note in case you're confused (because I didn't know how to fit an explanation in and you will need to know what's going on for the next couple sections):
Because Sherlock went and helped out John as a kid, things got screwed up (which is why we listen to the Doctor mister!). John never went into the military (having originally done it to spite his dad) and so never met Sherlock at St. Bart's. The rest is all effects from that. Sherlock died because he picked the wrong pill and John wasn't their to shoot the cabbie, so everybody thinks he's dead (having read it in the papers).
Yeah. I didn't know how to put this in the story. You can probably see why. I'm sorry if this makes no sense, it worked in my head though so hopefully it makes sense when it all comes out and isn't just gibberish...
