It follows one of your teammates.

You activate Crucible a little too late.

Your teammate gets killed by it.

You forgot to keep an eye on your other teammate.

You use Fountain of Renewal.

But you use it after your teammate dies.

Captain: Don't worry, they won't get the ace if they can't catch me.

Uses Combat Roll.

Gets stuck on wall.

Captain: ;-;

Proceeds to get rekt by the enemy team.
