"What do you have going on downstairs? ;)"
Or if you're planning on or already transitioning:
"Do you want 'the surgery'?"
Like, how hard is it to get that any question about what genitalia someone has is and can be invasive to some people, especially trans and nonbinary people?
Also in hospitals. I get that in some cases they need to know, but like, there's better ways to ask that question, and you don't need to know what my genitals look like in order to treat a broken toe.
