This is what Kevin Feige meant by "The first five minutes are going to be devastating." Tony will be witness to this, bringing his Age of Ultron vision to fruition. Then there will be a hard reset to before everyone dies and the events will play out differently. I think there will be a lot of resetting time by Dr. Strange in order to improve the outcome.
