Hearts can be gained in chatrooms, over text and sometimes in story mode. They can be traded for hourglasses and they strengthen your relationships with different characters.
In chatrooms, lots of hearts can be gained for different characters. This means that sometimes you can answer how you want and get a heart for whatever you answer with for different characters. For example, one answer might get you a heart for Zen while the other answer gets a heart for Seven.
It's better to know who you want to pursue/have the best relationship with from the start of the game so you can reply with the answer which is more favourable to the character you want to pursue in chatrooms so you're more likely to get their route and get a good ending. If you don't do this, you might realise that you want Yoosung's route but end up on Jaehee's route instead - which would be frustrating as you're stuck on Jaehee's route and can't have a relationship with Yoosung. So hearts are crucial in chatrooms.
Texts however, are a different matter. Yes, you can still get hearts but you don't get as many as you do in chatrooms. You get to reply once per conversation over text messages. And you don't actually HAVE to reply to texts. They're good if you want to strengthen your relationship with a specific person as you can reply to just them - unlike in chatrooms where there tends to be more than one person in the chat with you.
Overall, hearts play a big part in which route and ending you get so you should try to get as many as possible.
