You are Cas' fiancèe. When Lucifer had taken over Cas he pretended to be Cas and now you're pregnant with his child and since he is back in the cage he gets in your mind and comes in hallucinations.

"We can get married, I can find a way out of the cage, we can have a family, be normal, I'll stop trying to start an apocalypse, if it's a girl we can name her Mary and if it's a boy, John"Lucifer said and you gave a small broken chuckle as he mentioned the names your mom and dad.
