It's simply a form.
To officially have your character, you must first do a try out. Roleplay part of your character's past. If they're a Captured, roleplay what happened when they were caught. If they're a Turned, maybe you could do the first time they realized they were cats.
Roleplay Example:
BE. DETAILED. MAKE A COMPLICATED PERSONALITY AND A COMPLICATED DESCRIPTION! ITS NOT A COMMAND GAME!
