I think the meaning is well let me split it

Then we're all just fucked: I think when he says that he saying if you move out of your hometown and your heart doesn't feel happy or satisfied but when you come back to ur hometown ur heart is happy again and you're pretty much screwed because you got nowhere to go but ur home town to keep u happy at least.
