Let's be real. Derek would rather have died from appendicitis than go another day without seeing Hannah and Oliver.

I mean he already ignored it for way too long because he wanted to see them.

Maybe he was afraid to admit that he was sick because he was afraid that the others would worry about him. Which they did anyway.

Derek doesn't want his friends to worry about him. He can't stand to see them like that any more than they can stand seeing him in the hospital.
